Iit has been this way for me since release, I've uninstalled and reinstalled twice. I love this game and I'm not tripping to be negative but it happens most often on decapitation and inferno but has happend on every op. I'd have maxed everything twice over by now if it wasn't for the bug. I'll still play either way but I'd really like to not have this happen if it's not too much trouble

If uninstalling didn't help you might need to format your console, don't worry it doesn't delete your progress in any of your games, I don't remember exactly how it's done but it will make your console run better overall, you can most likely find more info about this on the xbox support site, hope this helps
